ACH ENROLLMENT FORM
If you would like to have your monthly maintenance charges automatically withdrawn from your bank account please provide Empire with the completed form and a voided check from the account you would like funds withdrawn from.
MOVE-IN / MOVE-OUT FORMS
All moving companies must submit their COI, Workers Compensation Certificate and Indemnification Form to Empire prior to scheduling the move. Completed documents can be emailed to contact@eispropertymanagement.com. All moving fees must be remitted by both the seller and the buyer to the Cooperative’s attorney at closing.
CERTIFICATE OF INSURANCE & CONTRACTOR AGREEMENT
All contractors and vendors preforming services on site must submit their COI, W-9, Westchester County License, Workers Compensation Certificate and Indemnification Form to Empire prior to preforming any work. Completed documents can be emailed to contact@eispropertymanagement.com.
RENOVATION PACKET
Any shareholder interested in preforming a renovation in their unit must complete and submit the renovation paperwork to Empire at the same time paperwork is submitted to the Town of Mamaroneck. The completed packet can be dropped off at or mailed to Empire’s office. All forms must be submitted in full, in hard copy, along with any applicable fees.
