CERTIFICATE OF INSURANCE & INDEMNIFICATION FORM
All contractors and vendors preforming services on site must submit their COI, W-9, Westchester County License, Workers Compensation Certificate and Indemnification Form to Empire prior to preforming any work. Completed documents can be emailed to contact@eispropertymanagement.com
RENOVATION PACKET
Any unit owner interested in preforming a renovation in their unit must complete and submit the renovation paperwork to Empire at the same time paperwork is submitted to the Village of Sleepy Hollow. Completed packet can be dropped off at or mailed to Empire’s office. All forms must be submitted in full, in hard copy, along with any applicable fees.
MOVE IN / MOVE OUT & DELIVERY FORMS
All moving and delivery companies must submit their COI, Workers Compensation Certificate and Indemnification Form to Empire prior to scheduling the move/delivery. Completed documents can be emailed to contact@eispropertymanagement.com.
